Certified Wound Care & RTA Prevention
Our certified wound care program is overseen by Apryll Southworth, LPN, CWCA. Our approach focuses not only on wound healing, but importantly on preventing wound-related return-to-acute ( Re-admission) events through early identification, close clinical monitoring, patient/caregiver education, and coordination with physicians and other providers.
Transitional Care Program – Focused on Preventing Re-admission
Our Transitional Care Program is another area where we believe SNHH can provide significant value to our community. The program is designed to bridge the gap between the acute care setting and the patient’s home, with the goal of preventing complications, avoiding unnecessary emergency department visits and hospitalizations, and reducing Re-admission. This type of intervention is particularly important in a value-based environment, where preventing avoidable utilization can directly improve both patient outcomes and health plan financial performance.
