With over 25 years of experience as a licensed Physical Therapist and two decades dedicated rehabilitation and home health care, Johnathan brings a wealth of clinical expertise, compassionate leadership, and a deep commitment to helping seniors thrive safely and independently in their own homes.

Johnathan joined Sierra Nevada Home Health with a mission to elevate the standard of in-home care by combining clinical excellence with personalized support. His passion for the senior community is at the heart of everything he does—whether developing care plans, mentoring staff, or working directly with clients and families.

Under Johnathan’s leadership, Sierra Nevada Home Health has become known for its attentive, patient-centered approach and high-quality home health services. His belief that every individual deserves dignity, safety, and independence drives his daily work and inspires the entire care team.

Noel Perryman RN, BSN
Finance Director


As Finance Director at Sierra Nevada Home Health, Noel brings over 22 years of financial leadership, with a strong focus on strategic planning, compliance, and operational efficiency. With a background in both corporate finance and health services administration, Noel ensures that the organization maintains financial strength while delivering high-quality, compassionate care to patients in their homes.

Noel is responsible for overseeing all aspects of financial management, including budgeting, forecasting, reimbursement, and financial reporting. She works closely with team to align financial goals with patient-centered outcomes, supporting sustainable growth and regulatory compliance in a rapidly evolving health care environment.

Prior to joining Sierra Nevada Home Health, Noel held executive financial roles in hospitality, where she led cost-saving initiatives and implemented systems that improved transparency and accountability across multiple departments.

Noel holds a degree in Business Administration and Accounting from the University of Nevada, Reno, and is passionate about leveraging data and finance to support care teams and improve patient experiences.

Nicole Hayes
Director of Nursing

With over 16 years of nursing experience and a deep commitment to compassionate, patient-centered care, Nicole serves as the Director of Nursing for Sierra Nevada Home Health. In this leadership role, she oversees the clinical team, ensures the highest standards of quality care, and develops personalized care plans that support patients in maintaining their health and independence at home.

Nicole began her career as a registered nurse in cardiac telemetry, branching out to out-patient surgery and neuro rehab facilities, quickly discovering a passion for home-based care. Over the years, she has held progressive roles in clinical leadership, blending hands-on experience with strong administrative and regulatory knowledge.

Known for her compassionate leadership style and dedication to nursing excellence, Nicole is a trusted advocate for both patients and staff. She plays a vital role in ensuring compliance with healthcare regulations, training and mentoring nursing staff, and fostering a culture of integrity, empathy, and clinical excellence.

She is a licensed Registered Nurse (RN), and is a certified advanced skills in wound and ostomy management (WSOC).

April Southworth LPN, CWCA
Director of Transitional Care

April Southworth is a skilled and compassionate nurse leading our team dedicated to ensuring safe, seamless transitions for patients moving between levels of care—whether from hospital to home, skilled nursing facility, or outpatient support. With 33 years of clinical experience, she specializes in care coordination, discharge planning, patient education, and reducing hospital readmissions.

Known for a proactive, patient-centered approach, April works closely with interdisciplinary teams to assess medical, psychosocial, and environmental needs, creating individualized transition plans that promote continuity of care and optimal recovery. April’s expertise includes managing chronic conditions, medication reconciliation, and supporting patients and families during vulnerable periods of change.

With a strong foundation in both acute and post-acute care settings, April is passionate about bridging gaps in the healthcare system, improving outcomes, and empowering patients to manage their health confidently after discharge.

She is a licensed practical nurse, (LPN), and is a certified wound care associate (CWCA), and consistently brings empathy, clinical insight, and advocacy to every patient interaction.

 

Priscilla
Intake and Scheduling Manager

Priscilla is an experienced and highly organized Home Health Intake and Scheduling Manager with a proven track record in streamlining care coordination, optimizing patient intake processes, and ensuring timely, efficient scheduling for home health services. With 5 years in the healthcare industry, she plays a critical role in connecting patients to the care they need—right when they need it.

In this role, Priscilla oversees the end-to-end intake process, including referral management, insurance verification, and initial patient assessment coordination. By working closely with physicians, case managers, and clinical teams, she ensures that all documentation is accurate and complete, facilitating a smooth transition into home health services.

With a deep understanding of home health regulations and EMR systems, Priscilla brings both compliance and compassion to every aspect of the job. She excels at balancing operational efficiency with a patient-centered mindset—ensuring every referral is treated with urgency, empathy, and attention to detail.

A natural communicator and problem-solver, Priscilla leads a scheduling team that coordinates care across multiple disciplines, ensuring clinicians are deployed effectively and patient needs are prioritized. Through her leadership, the intake and scheduling department consistently meets performance metrics, enhances patient satisfaction, and supports positive clinical outcomes.
